  • Vaccine Clinic November 5, 2020

    Clay County Health Department will be hosting a vaccine clinic by appointment only at Liberty North High School on November 5, 2020.  This clinic is only for students that are at least 16 years or older.  The clinic will be offering the reguired meningoccocal vaccine that students need prior to starting their senior year.  Influenza, HPV and meningitis B vaccines will also be available.  Transportation will be offered to students if needed.  Please contact the nurse at 736-7040 for more information.


    Health Record Update 

    Each year, every returning LPS student must have a Health Update Form completed by their parents. This form also gives consent for the nurse to administer Tylenol. (Link for the form is to the right.)  If your child will be taking medication other than Tylenol at school, please also complete a Medication Permission Form. If your child has a medical diagnosis of Asthma, Diabetes, Seizures, or a Severe Food Allergy please complete an Action Plan and obtain a physician signature.



    Junior Class

    Prior to starting their senior year all students must turn in proof of the required Meningococcal Conjugate (MCV4) immunization received after their 16th birthday to the school nurse. Proof of immunization may either be faxed or emailed to the nurse.  If you are unsure if your student still needs this vaccine or needs to turn in proof of vaccine please call the school nurse.
