    A Note from the DMS Nurse:

     

    Health Record Update:

    Each year, every returning LPS student must have a Health Record Update on file in the nurse's office. This form is completed as part of the online registration starting in July. In addition to updating health concerns, this form also allows parents to give consent for the nurse to administer acetaminophen (Tylenol) during the school year. If your child will be taking medication other than Tylenol at school please also complete a Medication Permission Form. If your child has a medical diagnosis of Asthma, Diabetes, Seizures, or a Severe Food Allergy please complete an Action Plan and obtain a physician signature.

     

    REQUIRED Vaccines for Incoming 8th graders (2022-2023):

    All incoming 8th graders (including virtual students) must submit proof of the required Tdap and Meningococcal Conjugate (MCV4) immunizations BEFORE the first day of the '21-22 school year. Students will not be allowed to return to school without proof of these immunizations on file. Records can either be emailed, faxed, or mailed to the nurse.

    All Grades: Updated immunization records can be submitted at any time during middle school.

    TYLENOL POLICY

    Regular strength Tylenol is available starting in the 6th grade and can be given ONLY with the signed permission waiver from the parent or guardian. Tylenol will be monitored and parents notified of frequent visits/requests for Tylenol.
